9/18/2023 EnchantaVaughn Approved Behavior: QMHPA arrived to the home of Ms. Broadnax. QMHPA informed Ms. Broadnax that this session was going to focus on her mental health. Before QMHPA began the session, QMHPA allowed Ms. Broadnax the opportunity to discuss with QMHPA any current situations that she felt needed to be addressed. Ms. Broadnax did not present any symptoms of S/I or H/I at the time of the session. Intervention: QMHPA began the session off by engaging Ms. Broadnax in a discussion surrounding her current emotional state. QMHPA began the session by addressing all relevant daily life skill activities, reinforced coping skills to reduce anxiety, release stress and manage depressive behavior patterns. QMHPA provided Ms. Broadnax with cognitive/behavioral modification techniques to improve self-esteem and strengthen self-determination to successfully achieve all treatment goal objectives. QMHPA educated Ms. Broadnax on the importance of assertive communication skills to ensure that the intervention process meets the expectations and provides the resources to improve Ms. Broadnax overall quality of life standards. QMHPA and Ms. Broadnax discussed utilizing time efficiently, using guidelines and structure to remember daily tasks. QMHPA provided Ms. Broadnax with communication skills to assist Ms. Broadnax with expressing her concerns proactively, develop a plan of the day, establish medication compliance and attend to all health management obligations. Response: Ms. Broadnax appeared to be in receptive to QMHPAs’ suggestions that were provided to her during this session. Ms. Broadnax compliantly engaged in all topic discussions without any negative oppositional defiant behavior. Ms. Broadnax required prompting to organize her weekly planner for all mental health related appointments. Ms. Broadnax spoke about her recent disagreement with her mother and how she effectively used positive communication skills to avoid a confrontation. Ms. Broadnax expressed to QMHPA that she has a difficult time dealing with her family and their many issues stemming from drug addiction and money problems then having to deal with her own battles. Ms. Broadnax indicated that she is aware that when she becomes frustrated, she tends to demonstrate down and isolate to cope with her anxiety. Ms. Broadnax receptively acknowledged all interventions presented to improve effective communication skills that will reduce tensions within her household. Ms. Broadnax shared that she is working hard to improve her daily life skills so that she can become more self-sufficient and not have to depend entirely on others for help. Plan: QMHP-A will assist the client in developing coping strategies (e.g., more physical exercise, less internal focus, increased social involvement, more assertiveness, greater need sharing, more anger expression) for feelings of depression; reinforce success 1-3 x weekly).
